Job summary

Premier Home Health Providers Inc. is seeking a per diem Physical Therapist in California to deliver skilled home health care, assess mobility, and help patients regain independence at home.

Responsibilities include evaluating functional status, creating treatment plans, educating families, coordinating with physicians, and supervising aides as needed. California license, CPR, and a valid driver’s license are required; 2 years of PT experience preferred, especially in home health.

Qualifications

• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification (AHA).
• Valid driver's license and reliable insured vehicle.
• Strong communication skills and ability to connect with patients and families.

Responsibilities

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop plans.
• Deliver PT services per physician's plan of care and CA rules.
• Educate patients and families on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise PTAs and oversee Home Health Aide care bi-weekly when PT is sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
• Perform other duties as required for patient services.
