Granite Peak Home Health is seeking a Physical Therapist to provide physician-prescribed PT in clients' homes across Montana. Applicants should hold an approved PT degree and Montana license, with BLS/CPR, HIPAA knowledge, and strong community outreach skills. Home health experience is preferred, and a clean driving record is required.

The role offers a flexible schedule, mileage reimbursement, and competitive pay DOE, with daily collaboration among a multidisciplinary home care team.

Formación

• Graduate from a physical therapist curriculum approved educational institution.
• Currently licensed in the state of Montana.
• 1 year home health experience, preferred.
• BLS/CPR required for this position.
• Knowledgeable with healthcare regulations, HIPAA compliance, and patient privacy requirements.
• Knowledge of the local healthcare market and community outreach strategies.

Responsabilidades

• Provides physician-prescribed physical therapy.
• Improves or minimizes residual physical disabilities of the patient.
• Returns the individual to optimum and productive level within the patient's capabilities.
• Participates with all other home care personnel in patient care planning.
• Directs and supervises personnel as required.
• Responsible for initial assessment, plan of care, maintenance program development, and modifications and reassessments every 30 days.
• Performs all skilled procedures as ordered by the physician.
• Consult with physicians regarding changes in treatment.
• Writes reports to physicians regarding patient's progress.
• Instruct patients and family/significant others in-home programs and activities of daily living.
• Attends all patient care conferences as scheduled.
• Prepares clinical and progress notes.
• Assists physician with evaluating level of function.
