Job summary

BAYADA Home Health Care in Arizona is seeking a dedicated Physical Therapist to join our home health visits team. This full-time PT role offers autonomy, meaningful patient relationships, and strong mentorship in a supportive, mission-driven environment.

You will assess mobility and safety, develop evidence-based plans, provide gait training and manual therapy, and document care in our EMR system. Weekly pay, flexible scheduling, paid training, and comprehensive benefits are included.

Qualifications

• Current Arizona PT license (or eligibility).
• Graduation from an accredited PT program.
• Comfort with EMR systems (training provided).

Responsibilities

• Perform mobility, strength, balance, and safety assessments.
• Develop individualized, evidence-based treatment plans.
• Provide gait training, manual therapy, balance work, and therapeutic exercise.
• Implement skin and wound-prevention strategies.
• Educate clients and families to promote long-term success.
• Document care using our intuitive EMR system.
