Job summary

PARS Therapy in Angleton, TX is seeking a compassionate Physical Therapist to join our home health team. The PT will assess patients at home, create individualized treatment plans, and support mobility and pain reduction in a home setting.

You'll educate patients and caregivers, document progress, and collaborate with an interdisciplinary team to ensure safe, effective care that improves quality of life for our clients.

Qualifications

• Active Physical Therapist license in Texas.
• Graduate degree in Physical Therapy from an accredited program.
• Professional liability insurance coverage in TX.
• Experience in home health or rehabilitative settings preferred.
• Strong communication and patient education skills.

Responsibilities

• Evaluate patients' physical abilities and functional limitations at home.
• Develop and implement individualized treatment plans to restore function.
• Provide exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans to achieve outcomes.
• Document evaluations, treatments, and progress in digital systems.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and recommend fall prevention.
• Deliver compassionate, patient-centered care to promote independence.
