Job summary

Palms-Home-Care seeks a skilled Home Health Physical Therapist to deliver individualized therapy in patients' homes in Ocala, Florida. You will assess, plan, and implement therapy to improve mobility and independence following illness, injury, or surgery.

The role requires a current PT license, CPR certification, and a driver’s license. Experience in home health and knowledge of OASIS and EMR systems like Kinnser is preferred.

Qualifications

• Current Physical Therapist license.
• Graduate of accredited PT program.
• 1-2 years of experience preferred in Home Health.
• Strong communication skills.
• Ability to work independently.
• Possesses and maintains current CPR certification.
• Valid driver's license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.

Responsibilities

• Perform initial evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, manual therapy, and mobility training.
• Assess patient progress and adjust care plans accordingly.
• Educate patients and caregivers on safety and exercises.
• Maintain timely and accurate documentation.
• Collaborate with interdisciplinary care teams.
• Follow home health and Medicare guidelines.
