Job summary

Palms-Home-Care in Florida is expanding and seeks a skilled Home Health Physical Therapist to deliver in-home therapy, improve mobility, and restore independence after illness or surgery. You will perform evaluations, create individualized plans, guide patients and caregivers on exercises, and collaborate with care teams while adhering to Medicare guidelines.

Qualifications

• Current Physical Therapist license required.
• Graduate of an accredited PT program.
• 1–2 years' home health experience preferred.
• Strong communication skills and independence.
• CPR certification required.
• Valid driver's license and reliable transportation.
• Familiarity with OASIS and Medicare guidelines.
• Experience with Kinnser EMR systems.

Responsibilities

• Perform initial evaluations and develop individualized treatment plans
• Provide therapeutic exercises, manual therapy, and mobility training
• Assess patient progress and adjust care plans accordingly
• Educate patients and caregivers on safety and exercises
• Maintain timely and accurate documentation
• Collaborate with interdisciplinary care teams
• Follow home health and Medicare guidelines
