UnitedHealth Group is seeking a Physical Therapist for Home Health in Ocala, Florida. You will assess patients’ functional status, develop and implement plans of care, and coordinate with a care team to improve mobility and pain outcomes.

The role requires active PT licensure in the state, a valid driver's license, and the ability to travel to patient homes. We offer a comprehensive benefits package and opportunities for growth within the Optum family.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification within 90 days of hire.

Responsabilités

• Evaluates the patient's functional status and therapy needs and assists in plan of care development.
• Provides PT services within scope of practice and according to plan of care.
• Observes and reports patient responses to treatment to supervising staff.
• Treats patients to relieve pain, improve function, and maintain maximum functioning.
• Instructs patient and family on plan of care and progress toward goals.
• Visits patients per Plan of Treatment and completes progress notes.
• Completes PT evaluations and develops plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
