Job summary

Pathways Healthcare is seeking a Physical Therapist for our Westwood, MA office with a service area including Dorchester and surrounding areas. The role is full-time, Monday through Friday, with a focus on home health assessments and in-home rehabilitation.

You will develop and implement therapy plans, monitor progress, and collaborate with physicians and family caregivers to support patient goals. A strong two-year background in PT and MA licensure are required.

Qualifications

• Must hold a degree in Physical Therapy from an accredited program.
• Active Physical Therapist license in Massachusetts.
• At least two years of relevant physical therapy experience; home health preferred.
• Strong verbal and written communication; solid organizational skills.
• Maintain current BLS CPR certification and a valid driver's license.

Responsibilities

• Deliver PT services per physician plan of care and state law.
• Assess functional status, mobility, gait, ROM, and home environment.
• Administer therapies and instruct on use and care of devices.
• Report changes in patient condition to physician promptly.
• Collaborate with care team, educate family and caregivers.
