Job summary

Freedom At Home is seeking a compassionate and motivated Physical Therapist (PT) to join our in-home care team. You will assess patients in their homes, develop individualized treatment plans, and deliver skilled therapy.

This role emphasizes collaboration with physicians and nurses, education for patients and caregivers, and timely completion of OASIS and clinical documentation. A Michigan PT license and a valid driver's license are required.

Qualifications

• Licensed Michigan Physical Therapist in good standing.
• Minimum 2 years PT experience preferred.
• Home Health experience is a plus.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans
• Provide skilled physical therapy services
• Educate patients and caregivers on exercises, safety, and mobility
• Complete OASIS and clinical documentation in a timely manner
• Collaborate with physicians, nurses, and other healthcare professionals
• Monitor patient progress and adjust treatment plans as needed
• Promote safe, high-quality patient care and excellent outcomes
