Job summary

Freedom at Home is seeking a compassionate Home Health Physical Therapist to evaluate patients in their homes and develop individualized treatment plans. You will provide skilled physical therapy services, educate patients and caregivers, and ensure timely OASIS documentation while collaborating with the care team.

Applicants should have graduated from an accredited PT program, hold a Michigan PT license, CPR certification, and at least 1 year of PT experience. Home Health experience is a plus.

Qualifications

Graduate of an accredited Physical Therapy program.
Current Michigan Physical Therapist license.
Current CPR certification.
Minimum of 1 year of PT experience.
Home Health experience is a plus but not required.

Responsibilities

Evaluate patients in their homes and develop individualized treatment plans.
Provide skilled physical therapy services.
Educate patients and caregivers on exercises, safety, and mobility.
Complete OASIS and clinical documentation in a timely manner.
Collaborate with physicians, nurses, and other healthcare professionals.
Monitor patient progress and adjust treatment plans as needed.
Promote safe, high-quality patient care and excellent outcomes.
