Job summary

PARS Therapy in Cypress, TX is seeking a compassionate Physical Therapist to join our home health team. You will assess patients in their homes, design and implement individualized treatment plans, and help reduce pain and improve mobility.

You will educate patients and caregivers, document progress using digital platforms, and collaborate with physicians, nurses, and occupational therapists to support safe, effective care.

Qualifications

• Active Physical Therapist license in Texas.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Excellent communication skills and a friendly, professional demeanor.
• Up-to-date knowledge of evidence-based treatment techniques.

Responsibilities

• Evaluate patients' physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans as necessary to achieve optimal outcomes.
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and make recommendations to prevent falls or re-injury.
• Deliver compassionate care while empowering patients to achieve greater independence.
