Job summary

PARS Therapy is seeking a compassionate Physical Therapist (PT) to join our home health team serving Deer Park, TX. You will assess patients in their homes, design individualized treatment plans, and guide mobility, pain reduction, and independence.

Ideal candidates have an active Texas PT license, graduate degree in Physical Therapy, and experience in home health or rehab settings. Strong communication and evidence-based care are essential, with flexible scheduling offered.

Qualifications

• Active Texas Physical Therapist license required.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to patient needs and maintain evidence-based practice.

Responsibilities

• Evaluate patients in home health settings and assess functional abilities.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatments, and progress using digital platforms.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and recommend fall-prevention measures.
• Deliver compassionate, patient-centered care to promote independence.
