Qualifications

Minimum 1–2 years of experience, preferably in home health.
Current CPR certification required.
Valid driver's license, reliable transportation, and auto insurance needed.

Responsibilities

Evaluate and assess patients’ functional status and rehabilitation potential.
Develop individualized care plans in collaboration with the team.
Provide hands-on therapy and educate patients on treatment plans.
Monitor patient progress and adjust treatment plans as needed.
Supervise Physical Therapist Assistants and provide guidance.
