Job summary

Nursing & Rehab At Home is seeking a licensed physical therapist to deliver PT services at patient homes under physician plan of care. The role includes evaluating functional status, directing therapy, and educating families. You will complete OASIS assessments, coordinate care, and participate in discharge planning.

Supervisory duties include overseeing PT assistants in accordance with policy. This position emphasizes collaboration with clinical managers and home health teams to ensure quality

Qualifications

• Provides physical therapy services according to physician's plan of care.
• Completes OASIS assessments as assigned.
• Acts as case manager and coordinates patient care.
• Identifies needs for other home health services.
• Prepares clinical progress summaries and discharge planning.
• Supervises PT assistants per policy and state regulations.

Responsibilities

• Provide PT services per physician plan of care and assess functional status.
• Assess muscle strength, mobility, ROM, and rehab potential.
• Instruct patients and families in use and care of therapeutic appliances.
• Coordinate patient care and discharge planning.
• Supervise PT assistants and educate home health staff.
