Job summary

Concierge Home Care is seeking a Home Health Physical Therapist to evaluate patients in their homes, design and implement mobility programs, and support recovery with patient and caregiver education. You will collaborate with physicians and the care team, supervise PTAs, and document visits using AI-assisted tools.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Valid driver’s license and reliable transportation.
• Willingness to work in patients’ homes and manage own schedule.

Responsibilities

• Perform patient evaluations and develop treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on recovery and safety.
