Concierge Home Care is seeking a Home Health Physical Therapist to provide skilled therapy in patients’ homes, focusing on mobility, strength, and independence. You’ll manage evaluations, treatment plans, and patient education while collaborating with physicians and care teams.

Qualifications

• Active Florida Physical Therapist license required.
• Valid driver’s license and reliable transportation.
• Willingness to work in patients’ homes and flexible scheduling.

Responsibilities

• Conduct patient evaluations and develop treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on recovery and safety.
• Collaborate with physicians and interdisciplinary care teams.
• Supervise and support Physical Therapy Assistants (PTAs).
• Document visits efficiently using AI-assisted tools.
