Job summary

Adoration Health in Ringgold, GA is seeking a passionate Home Health Physical Therapist to join our team, delivering patient-centered care in patients' homes. You will perform evaluations, establish goals, and develop treatment plans while coordinating with physicians, patients, and families.

Qualifications

• Graduate of a Physical Therapist Education program approved by CAPTE.
• Current Physical Therapist licensure in State and CPR certification required.
• Two years of experience in adult physical therapy, home health experience preferred.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.

Responsibilities

• Perform PT evaluation and treatment of patients based on physician orders, including adaptive equipment.
• Establish measurable goals and develop treatment plans to move patients toward independence.
• Maintain communication among treatment team, physician, patient, and family members as applicable.
• Maintain thorough and timely documentation of services for proper billing.
• Supervise PTAs and PT Aides in direct patient care as applicable.
