Job summary

BrightStar Care of Camden & Cherry Hill is seeking a licensed Physical Therapist to perform home health PT evaluations and provide follow-up visits per physician orders. You will develop and adjust individualized plans of care and deliver therapy for gait, balance, mobility, strength, and fall prevention in patients’ homes.

Ideal candidates have NJ PT licensure, CPR/BLS, 1+ year home health experience, reliable transportation in the Camden/Cherry Hill area, and strong documentation and patient

Qualifications

• NJ Physical Therapy license in good standing
• 1+ year of PT experience (home health preferred)
• CPR/BLS certification
• Reliable transportation within Camden/Cherry Hill
• Strong documentation and patient communication skills

Responsibilities

• Perform PT evaluations and follow-up visits per physician orders
• Develop and adjust individualized plans of care
• Deliver therapy for gait, balance, mobility, strength, and fall prevention
• Train patients and caregivers on home exercises and safe mobility strategies
• Document per NJ DOH, Medicare, and BrightStar standards
• Collaborate with RNs, OTs, STs, and CHHAs
