Job summary

Home Sweet Home Health is seeking a licensed Physical Therapist to provide one-on-one home health care in the North Scottsdale area. The role emphasizes flexible, per-visit compensation and autonomy to tailor treatment plans to patients' goals.

You will evaluate and treat patients in their homes, develop individualized plans, document visits, and collaborate with physicians and care teams. A sign-on bonus of $5,000 is offered to qualified candidates.

Qualifications

• Active Arizona Physical Therapist license.
• CPR certification (AHA).
• Minimum 1 year of clinical experience (home health preferred).
• Strong clinical judgment and time management.
• Reliable transportation, valid driver’s license, and insurance.
• Strong communication and a patient-first approach.

Responsibilities

• Conduct in-home evaluations, start of care, and follow-up visits.
• Develop and implement individualized treatment plans.
• Improve strength, balance, mobility, and functional independence.
• Educate patients and caregivers on safety and equipment use.
• Document visits using WellSky Kinnser EMR.
• Collaborate with physicians, RNs, and the care team.
