Job summary

Home Sweet Home Health is seeking a licensed Physical Therapist for home health in the Phoenix West Valley region. You will provide one-on-one, in-home care to help patients regain strength, mobility, and confidence in daily life.

Per diem, part-time, and full-time schedules are available, with a sign-on bonus of $5,000. Flexible scheduling and a steady patient base support productivity and independence in care.

Qualifications

Active Arizona Physical Therapist license required
CPR certification (AHA) required
Minimum 1 year of clinical experience (home health preferred)
Strong clinical judgment and time management
Reliable transportation, valid driver’s license, and insurance
Strong communication and a patient-first approach

Responsibilities

Conduct in-home evaluations, start of care, and follow-up visits
Develop and implement individualized treatment plans
Improve strength, balance, mobility, and functional independence
Educate patients and caregivers on safety and equipment use
Document visits using WellSky Kinnser EMR
Collaborate with physicians, RNs, and the care team
