Home Sweet Home Health is seeking a licensed Physical Therapist to provide one-on-one home health care in the North Scottsdale area. You will evaluate patients in their homes, initiate and adjust personalized treatment plans to restore strength, balance, and independence.

The role emphasizes flexible scheduling, autonomous work, and meaningful impact on patient safety at home. Arizona licensure, CPR, and strong communication skills are essential for success.

Qualifications

• Active Arizona Physical Therapist license.
• CPR certification (AHA).
• Minimum 1 year of clinical experience in home health preferred.
• Strong communication and time management.
• Reliable transportation, valid driver’s license, and insurance.
• Strong communication and a patient-first approach.

Responsibilities

• Conduct in-home evaluations, start of care, and follow-up visits
• Develop and implement individualized treatment plans
