Job summary

Home Sweet Home is seeking a licensed Physical Therapist to provide one-on-one home health care in the North Phoenix area. You will assess, treat, and advance mobility in patients’ homes, using WellSky Kinnser EMR for documentation, with a productivity-based compensation model and a $5,000 sign-on bonus for full-time positions.

The role offers flexible schedules (Per Diem, Part Time, Full Time) and the chance to impact patients’ independence while collaborating with a skilled care team.

Qualifications

• Active Arizona Physical Therapist license.
• CPR certification (AHA).
• Minimum 1 year of clinical experience (home health preferred).
• Reliable transportation, valid driver’s license, and insurance.
• Strong communication and a patient-first approach.

Responsibilities

• Conduct in-home evaluations, start of care, and follow-up visits.
• Develop and implement individualized treatment plans.
• Improve strength, balance, mobility, and functional independence.
• Educate patients and caregivers on safety and equipment use.
• Document visits using WellSky Kinnser EMR.
• Collaborate with physicians, RNs, and the care team.
