Qualifikationen

• Must possess and maintain valid CPR certification.
• Must be a licensed driver capable of traveling to all client locations.

Aufgaben

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations per agency policy; selects diagnostic materials for assessment.
• Informs physician of assessment results and recommended interventions before starting care.
• Develops and documents treatment plans with appropriate intervention approaches and priorities.
• Delivers organized patient treatment, including advising, educating and counseling patients.
• Presents patient information to physicians and keeps the team informed of status and plan.
• Provides treatment within patient needs and agency/payer parameters.
• Sets objective, measurable goals in collaboration with patients and team.
• Meets agency productivity standards and uses work hours efficiently.
