Job summary

Upham’s Home Health Services, a division of Upham’s Community Care, seeks a full-time Physical Therapist to provide rehabilitation services to patients in a home health setting in the greater Boston area.

The position offers a flexible schedule, comprehensive orientation, and ongoing mentoring. New physical therapy graduates are welcome to apply, with required MA license and CPR certification, and travel to patient homes as needed.

Qualifications

• Graduate of a physical therapy curriculum/program.
• Valid Massachusetts physical therapy license in good standing.
• Valid CPR certification.

Responsibilities

• Provide physical therapy in home health settings per physician prescriptions.
• Instruct and educate patients and family members on rehabilitation programs.
• Communicate progress with attending physicians or clinics.
• Coordinate with nursing staff and community agencies to meet patient needs.
• Attend monthly interdisciplinary staff meetings.
