Job summary

Enhabit Home Health & Hospice Careers is seeking a licensed Physical Therapist for our home-based care team in Colorado. The PT will examine and treat patients to improve mobility and functional independence, using specialty programs to support falls prevention and joint replacement recovery.

A driver’s license is required for community visits, with competitive benefits and ongoing professional development.

Qualifications

• Current PT license in the practicing state.
• At least 1 year of clinical experience preferred.
• Valid driver’s license and automobile liability insurance.
• Knowledge of modern therapy practices and modalities.

Responsibilities

• Evaluate and treat patients to improve mobility and daily living activities.
• Utilize specialty programs such as Balance and Fall Prevention and Total Hip/Knee/Shoulder programs.
