Job summary

Enhabit Home Health & Hospice in Colorado is seeking a Physical Therapist to assess and treat patients in the home setting, helping restore mobility and independent self-care. You will work under the branch director's supervision, using physical modalities and specialty programs to support patients' daily living activities.

The role requires 4–10 hour shifts with weekends on a rotating basis, a valid state driver’s license, dependable transportation, and insurance.

Qualifications

• Requires valid state driver’s license.
• Must have dependable transportation and automobile liability insurance.
• Able to drive an automobile in all types of weather conditions.
• Currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments in the home setting.
• Utilize physical modalities and specialty programs to improve mobility and daily living activities.
• Assist patients under physician orders and plan of care, with supervision of Branch Director.
