Job summary

Enhabit Home Health & Hospice is seeking a dedicated Physical Therapist to evaluate and treat patients in the home setting, aiming to improve mobility and independence in daily activities.

The role involves delivering skilled care under physician orders, teaching families and care teams, and coordinating with a Branch Director. Specialty programs include Balance and Fall Prevention, Spine Safety, and joint replacement programs. Valid licensure and reliable transportation are required.

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients to improve mobility and daily living.
• Perform skilled care per physician orders and plan of care.
• Educate family and health care team members on care plans.
