Job summary

A leading home health service provider in California is seeking a Physical Therapist Case Manager. This role involves evaluating patients in their homes, developing care plans, and assisting patients in regaining mobility. Ideal candidates will have a California Physical Therapist License and strong communication skills. Full-time position with competitive salary range of $120,000 to $135,000 plus bonuses and comprehensive benefits.

Qualifications

• Experience in home health care is highly preferred.
• Understanding of how illness/injury affects individuals.
• Experience working with an interdisciplinary team is highly desired.

Responsibilities

• Develop a care plan and work with PTAs to follow it.
• Help patients with mobility and regaining independence.
• Communicate with patients’ physicians for high-level care.
