Job summary

Adara Home Health is seeking a Physical Therapist Case Manager to oversee home health services in the community. You will collaborate with clients, families, and the health care team to implement goal-centered plans, while providing skilled care and clinical oversight to staff.

Reports to the Clinical Supervisor and travels to client homes from Blaine, MN to Roseville and nearby areas. Requires MN PT license, TB test, valid driver’s license, and clean background.

Qualifications

• Licensed PT in Minnesota with no restrictions.
• Proof of negative TB testing.
• Access to a dependable automobile for travel between stops.
• Valid driver's license and auto insurance when using a personal vehicle.
• U.S. Citizenship or valid Alien Work Permit.
• No disqualifying criminal history related to health services.
• Pass background checks as required by state/federal authorities.

Responsibilities

• Reflects the company’s mission and values in professional relationships.
• Performs initial and periodic assessments to determine care needs and evaluate outcomes.
• Collaborates with physicians and the home health team to update the care plan at least every 60 days.
• Advocates for client safety and reports any abuse or exploitation.
• Keeps accurate documentation and complies with state/payer requirements.
• Provides education to clients, families, and care staff.
• Delivers direct care within Minnesota PT Practice Act.
• Coordinates care with family, community resources, and other health care members.
• Provides clinical oversight and leadership to licensed and unlicensed staff.
• Manages financial aspects of home health services.
• Maintains schedule flexibility and adaptability.
