Job summary

Nursing & Rehab At Home is seeking a licensed Physical Therapist to deliver home-based care across our service area. You’ll evaluate and treat patients in their homes, collaborate with families, and follow physician plans of care.

This clinician-owned agency emphasizes autonomy, ongoing case management, and relationships. A degree in physical therapy, active CA license, CPR, and clean driving record are required; home health or community experience is a plus.

Qualifications

• Degree in physical therapy from an accredited program.
• Active California Physical Therapy Board license.
• Current CPR certification.
• A valid driver's license and an insured, reliable vehicle.
• Strong communication and organizational skills.
• One or more years of PT experience is preferred.
• Home health or community-based experience is a plus.

Responsibilities

• Evaluate and treat patients at home. Assess strength, mobility, gait, and range of motion; evaluate home safety and rehab potential.
• Deliver treatment under a physician's plan of care.
• Empower patients and caregivers by teaching use of therapeutic equipment and goals of therapy.
• Set treatment priorities, track progress, and inform physicians of changes in condition.
• Coordinate care and plan discharges; connect patients with home health services.
• Complete OASIS assessments as assigned.
• Collaborate and mentor; supervise PT assistants and oversee home health aides as needed.
