Job summary

Residential Home Health and Hospice is hiring a Physical Therapist to provide skilled home health care in the Du Quoin area. The role includes a $10,000 first-year bonus and ongoing opportunities for growth within a nationally recognized home health provider.

You will perform patient evaluations, develop therapy plans to improve strength and balance, and educate patients and caregivers on safe mobility. Travel to patient homes is required.

Qualifications

• Active Physical Therapist license in the state you work
• Minimum one year of PT experience delivering patient care
• Field-based home health requires a valid driver's license and 20% travel to patient locations

Responsibilities

• Deliver skilled, patient-centered home health care in patients' homes
• Perform comprehensive evaluations and develop therapy plans to improve strength, balance and independence
• Educate patients and caregivers on exercises, injury prevention and safe mobility
• Maintain compliant EMR documentation
• Work independently while collaborating with an interdisciplinary team
• Travel to patient homes within the assigned territory
