Qualifications

• Active Physical Therapist license in the state you work.
• One year of PT experience delivering home health care.
• Valid driver's license and ability to drive to patient locations (~20%).

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Conduct comprehensive patient evaluations and develop therapy plans to improve strength and balance.
• Educate patients and caregivers on exercises, safety, and recovery at home.
• Perform thorough EMR documentation and think critically in real time.
• Work independently, but stay connected with the team and travel to patient homes.
