Job summary

Graham Healthcare Group seeks a Physical Therapist (PT) to provide skilled home health care in Lisle, IL and surrounding areas. You will evaluate patients, develop therapy plans, and educate families to support recovery and independence.

Qualifications

• Active Physical Therapist license in the state you work in.
• Minimum of 1 year of PT experience delivering skilled care.
• Valid driver’s license and ability to drive ~20% of the workday.

Responsibilities

• Deliver skilled, patient-centered home health care in patients' homes.
• Perform evaluations, mobility assessments, and create therapy plans.
• Educate patients and caregivers on exercises, safety, and recovery.
• Maintain compliant EMR documentation and teamwork with the interdisciplinary group.
• Work independently while staying connected to the team and travel to patient homes.
