Job summary

Choice Health at Home in Boulder, CO is seeking a Physical Therapist to deliver comprehensive home health services. You will evaluate patients, create and implement personalized treatment plans, and document progress while collaborating with the care team.

Requirements include an accredited PT program and state licensure, CPR certification, and a valid driver’s license. A minimum of 1 year home health experience is preferred. This is a full-time role with a $5,000 sign-on bonus.

Qualifications

• Accredited physical therapy program completed and state licensure exam.
• Current Physical Therapy license in good standing.
• 1 year of home health experience preferred.
• CPR certification and valid driver's license required.

Responsibilities

• Perform and document an initial exam to identify problems and diagnoses.
• Plan and carry out individualized physical therapy programs to improve function.
• Evaluate progress and adjust treatment to maximize benefit.
• Maintain infection control and accurate documentation.
• Communicate effectively with patient, family and health team.
• Support agency policies and participate in care coordination.
