Job summary

Optimal Home Care in Colorado is seeking a licensed Physical Therapist to deliver in-home health services to our patients. The role offers a $10,000 sign-on bonus and requires a current Colorado license and strong clinical skills.

You will assess, treat, and discharge patients, document care plans, and collaborate with physicians and the care team to maintain high quality outcomes. Travel between patient homes is expected; mileage reimbursement and ongoing CE opportunities are available.

Qualifications

• Holds an active license to practice Physical Therapy in Colorado.
• Preferable at least one year of experience as a Physical Therapist.
• Strong written and verbal communication skills.
• Good interpersonal skills required.

Responsibilities

• Complies with ethical and professional standards of conduct as set forth by the American Physical Therapy Association.
• Assists the Physician in evaluating client/patients by applying diagnostic and prognostic tests and participates in the Physician’s Plan of Care.
• Develops, documents, implements and re-evaluates a Plan of Care noting all treatment modalities and frequencies.
• Treats patients to relieve pain and/or develop or restore function to maintain maximum performance.
• Reports to the Physician the client/patient’s reaction to treatment and/or changes in condition and documents progress notes weekly.
