Qualifikationen

• Graduate of an accredited Physical Therapy program.
• Current and valid Physical Therapist license in the state of practice.
• One year of home health experience preferred.

Aufgaben

• Perform comprehensive initial evaluations to identify functional limitations and establish therapy goals.
• Develop and implement individualized plans of care to improve mobility, strength, balance, and endurance.
• Provide therapeutic interventions to relieve pain and prevent disability.
• Evaluate patient progress and adjust treatment plans to achieve the best outcomes.
• Educate patients and caregivers on exercises, mobility techniques, and safety.
• Collaborate with physicians and interdisciplinary care team.
• Maintain clinical documentation within company guidelines.
