Job summary

Choice Health at Home is seeking a full-time Home Health Physical Therapist in Brighton, CO. You will deliver personalized rehabilitative care in patients’ homes, focusing on improving mobility, strength, and independence. A $5,000 sign-on bonus is offered, with a salary range of $110,000 to $125,000 annually.

Licensed PT in the state, CPR, and a valid driver's license are required. Home health experience is preferred.

Qualifications

Graduate of an accredited Physical Therapy program.
Current and valid Physical Therapist license in the state of practice.
One year of home health experience preferred.
Current CPR certification.
Valid driver's license.
Homecare Homebase (HCHB) experience preferred.

Responsibilities

Perform comprehensive initial evaluations to identify functional limitations and establish goals.
Develop and implement individualized care plans to improve mobility, strength, balance, and function.
Provide therapeutic interventions to relieve pain and improve independence.
Evaluate progress and adjust treatment plans to achieve outcomes.
Educate patients and caregivers on exercises, mobility, and safety.
Collaborate with physicians, nurses, and therapists as part of the team.
Maintain infection control standards and accurate documentation.
Ensure timely and quality clinical documentation within guidelines.
