Job summary

UnitedHealth Group is seeking a Physical Therapist in Home Health to evaluate patients, develop and implement a plan of care, and coordinate with physicians and the health care team to improve mobility and function. The position offers a $5,000 bonus for full-time, with a 4-day work week option and a competitive hourly rate.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient’s functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports patient response to treatment to supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs care team, patient, and family regarding plan of care and progress.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
