Job summary

UnitedHealth Group is seeking a licensed Physical Therapist for Home Health to assess patient needs and determine a treatment plan within the home. You will perform interventions, monitor progress, and coordinate with the care team to ensure optimal patient outcomes.

The role requires PT licensure, CPR, reliable transportation, and ability to work with diverse patients. Hourly pay ranges from $34.23 to $61.15, with a $5,000 bonus and a 4-day work week option.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient’s functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs health care team, patient, and family regarding plan of care and progress.
• Visits patients according to Plan of Treatment and submits progress notes.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
