Job summary

The role offers a $5,000 bonus for full-time positions and a 4-day workweek option, with compensation based on local labor markets and experience.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of client characteristics.

Responsibilities

• Evaluates patient’s functional status and PT needs; assists in plan of care development.
• Provides PT services within scope of practice and plan of care.
• Monitors patient response to treatment and communicates changes to healthcare team.
• Treats patients to relieve pain and improve function; maintains maximum functioning.
• Instructs patient and family regarding plan of care and progress.
• Documents progress notes after visits and submits per policy.
• Develops PT plan of care within state guidelines and reports outcomes.
