Job summary

UnitedHealth Group in the United States is seeking a Physical Therapist in Home Health. You will assess patients, determine a treatment plan, and implement interventions to improve mobility, endurance, pain, and overall well-being in home settings.

Responsibilities include evaluating functional status, developing the plan of care with the physician, delivering therapy within state law, and documenting progress notes.

Qualifications

• Current Physical Therapy licensure in state of practice
• Current CPR certification
• Current driver’s license and vehicle insurance or access to dependable transportation
• Ability to function in any home situation regardless of client demographics

Responsibilities

• Evaluates patient’s functional status and PT needs; assists in development of the plan of care
• Provides PT services within applicable state laws and plan of care
• Observes and reports patient response to treatment to nurse/physician
• Treats to relieve pain, restore function, maintain maximum functioning
• Instructs patient and caregivers about plan of care and progress
• Visits patients per Plan of Treatment; completes progress notes and submits them
• Completes evaluations and develops PT plan of care per state guidelines
• Reports outcomes of evaluation, goals, and frequency of care
