Job summary

UnitedHealth Group, part of Optum, is seeking a Physical Therapist in Home Health to assess and treat patients in their homes, develop treatment plans, and coordinate with physicians and nurses to improve patient outcomes.

The role emphasizes functional mobility, endurance, and pain management, with the flexibility of a 4-day work week option and a $5,000 bonus for full-time employment.

Qualifications

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Current driver's license and access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws.
• Observes and reports patient responses to treatment to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and/or maintain maximum functioning.
• Instructs patients, families, and care team about the plan of care and progress.
• Visits patients according to Plan of Treatment and submits progress notes.
