Job summary

Alternate Solutions Home Health, LLC is seeking a Physical Therapist I to provide skilled PT services in patients’ homes, ensuring safe, effective care under physician orders. You will educate patients and families, complete documentation, and maintain compliance with regulations in a dynamic post-acute setting.

Responsibilities include performing OASIS assessments, developing plans of care, and coordinating with agencies, schedulers, and care team.

Qualifications

• Active Physical Therapist license in the state of employment.
• Minimum one year PT experience; home health preferred.
• Ability to communicate effectively with patients, families, physicians, and co-workers.
• Detail-oriented, reliable, and able to prioritize tasks.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled PT services in patients' homes as ordered by the physician.
• Perform OASIS assessments to develop care plans.
• Develop and update plans of care; document progress.
• Educate patients and families on disease, treatment, and home exercise.
• Coordinate admissions, transfers, and discharges and communicate with team.
• Supervise PTAs and home health aides as applicable.
