Redcrest Home Health LLC is seeking a Physical Therapist I to provide skilled PT services in patients' homes under physician orders. The role includes educating patients and families, ensuring safety, and maintaining compliant documentation.

The PT will perform OASIS assessments, develop plans of care, and collaborate with the care team to promote positive outcomes and reduce hospitalizations. Requires active state license and reliable transportation.

Qualifications

• Ability to effectively communicate with patients, families, physicians and co-workers.
• Knowledge of physical therapy best practices and patient safety.
• Detail-oriented and observant in documentation and care planning.

Responsabilités

• Provides skilled physical therapy in patients' homes as ordered by the physician.
• Observes and reports patient progress and changes to the physician and agency staff.
• Performs assessments to develop and adjust an individualized plan of care.
• Develops and executes plans of care to restore function and relieve pain.
• Instructs patients and families on proper use of equipment and home care.
