BAYADA Home Health Care is seeking an experienced Physical Therapist for a PRN/Part Time role delivering home health visits in the Williamsburg, VA area. You will be part of a multidisciplinary team providing rehabilitative care and skilled nursing to adult and geriatric clients at home.

Responsibilities include conducting assessments, developing individualized care plans, gait and balance training, and documenting care using a state-of-the-art tablet.

Qualifikationen

• Current Virginia Physical Therapist license (PT).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.

Aufgaben

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance, safety).
• Implement wound care and antiseptic interventions using evidence-based dressings.
• Develop and implement individualized care plans including manual therapy and gait training.
• Continuously assess and revise the PT care plan with nursing in the team.
• Educate clients and their families on rehabilitative care to promote health and independence.
• Accurately document observations and interventions using EMR/tablet.
