BAYADA Home Health Care is seeking a Home Health Physical Therapist in Glen Ridge, New Jersey to provide skilled PT services in clients' homes across Essex County. The role requires one year of PT experience and NJ licensure, with an emphasis on evidence-based, patient-centered rehabilitation.

As an integral member of a multidisciplinary team, you’ll assess, plan, and implement customized care, document progress using EMR, and support clients to live safely at home with optimal function.

Qualifications

• One year of clinical PT experience required.
• Licensed Physical Therapist in New Jersey.
• Graduation from an APTA-approved PT program.

Responsabilités

• Make home visits to clients in designated geographic territories.
• Perform mobility, gait, balance, and safety assessments.
• Develop and implement individualized care plans with therapeutic exercises.
• Educate clients and families on rehabilitative care and activities.
• Document observations and interventions using EMR tablet.
