Responsibilities include home visits, diagnostic assessments, wound care, care-plan development, patient and family education, and precise documentation. NJ license required.

Formación

• Current NJ Physical Therapist license.
• Graduation from an APTA-approved program or equivalent with transcripts or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; experience with EMR preferred.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements on mobility, balance, strength, and safety.
• Implement wound care and protective interventions using evidence-based dressings.
• Develop and update individualized care plans with PT interventions.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions using EMR/tablet.
