BAYADA Home Health Care in Riverton, NJ is seeking a Home Health Physical Therapist to provide high-quality one-on-one care in patients' homes for both home health and outpatient visits. The role combines field visits with documentation and collaborative care planning in a senior living office and surrounding counties.

The ideal candidate holds an NJ PT license, has completed an APTA/AMA-approved program, and can manage time independently while delivering compassionate, evidence-based therapy.

Formación

• Current NJ Physical Therapist license.
• Graduate from an APTA- or AMA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility and gait, and conduct safety assessments.
• Develop and implement individualized care plans including manual exercises and gait training.
• Revise the PT care plan with nursing in the multidisciplinary team as needed.
• Educate clients and families on rehabilitative care and independent living.
• Accurately document observations and interventions using an EMR/tablet.
