Job summary

Excell Home Care and Hospice in Oklahoma City is seeking a Physical Therapy Assistant to provide PT services under the direction of a licensed Physical Therapist and per the physician’s plan of care.

You will work in patients' homes across the service area, guiding therapeutic exercises, using modalities as directed, documenting care, and collaborating with the home health team to support recovery and comfort.

Qualifications

• Licensed PTA in the state where the home health agency operates.
• Graduated from a CAPTE-accredited PTA program and passed the national exam.
• Maintains current CPR certification.

Responsibilities

• Provide direct PTA services under the direction of a physical therapist and per the plan of care.
• Treat patients to relieve pain, develop or restore function, and maintain maximum performance.
• Direct and aid patients in active and passive therapeutic exercises, gait and functional training.
• Utilize physical agents (heat, cold, water, light, electricity, ultrasound, massage).
• Observe and report patient responses and changes to the PT and organization staff.
• Instruct patients in use of assistive devices (wheelchairs, braces, crutches, canes, prosthetic/orthotic devices).
• Maintain clinical notes and patient records within prescribed timeframes.
• Participate in home care team meetings and ongoing staff education.
• Perform other duties as assigned.
