Job summary

CARE FIRST HOME HEALTH CARE INC is seeking a Physical Therapy Assistant to provide PT services in a home health setting under the direction of a licensed physical therapist. The role involves carrying out the physician's plan of care and delivering direct patient care in clients' homes.

Responsibilities include guiding patients through therapeutic exercises, supervising gait and transfer activities, and using therapeutic modalities as ordered.

Qualifications

• Graduate of an accredited physical therapy assistant program.
• Current certification or license as required by state law.
• Demonstrates good verbal and written communication and organization skills.
• Possess and maintain current CPR certification
• Licensed driver with automobile in good working order and insured.

Responsibilities

• Provides direct physical therapy according to the physical therapist's directions and plan of care.
• Treats patient to relieve pain, develop or restore function, and maintain maximum performance.
• Aids patients in active and passive therapeutic exercises, gait and functional training, transfer activities, and prosthetic training.
• Utilizes physical agents (heat, cold, water, light, electricity, ultrasound, massage).
• Observes and reports patient reactions to treatment and changes in condition to the PT and organization staff.
• Instructs patients in care and use of mobility devices.
• Maintains clinical notes and records for patient files.
• Participates in Home Care Team meetings and Inservice Education.
• Performs other duties as assigned.
